診察を受けたいのですが。受付はどこにありますか？
受診の受付は向こうですが、先にこちらでいくつかお伺いします。 当院に来られたのは初めてですか。
The reception desk is over there, but I would like to ask you a few questions first. Is this your first visit to our hospital?
Yes, it is. It’s also the first time I’ve seen a doctor in Japan.
はい、そうです。日本で病院にかかるのも初めてなんです。
保険証はお持ちですか。 それから、受診を希望される診療科はお決まりですか。
Do you have an insurance card? Do you know which department you would like to visit?
Yes, I have it here. I’ve had terrible diarrhea and vomiting and a serious stomach ache since yesterday, so I’d like to go to the internal medicine department.
はい、これです。昨日からひどい下痢と嘔吐、そして激しい腹痛があり、内科にかかりたいと思います。
わかりました。では、この診療申込書にお名前、ご住所を記入してください。 「受診を希望する診療科」の欄は「内科」に○を付けてください。それから、1 番の初診窓口の前にある機械で受付番号札を取って、お待ちください。 窓口で番号が呼ばれたら、番号札、診療申込書と保険証を出してください。
OK. Then, please fill in your name and address on this application form. Please circle “Internal Medicine” under “Department you wish to visit”. Then, please go to the machine in front of the First-Time Patient counter 1 and get a number ticket. When your number is called at the counter, please present your number ticket, application form, and insurance card.
初診窓口で受付をして、書類を受け取ったら、内科の受付に行ってください。
Once you have checked in at the First-Time Patient counter and received your documents, please go to the reception desk for the department of internal medicine.

I’ve been vomiting and had diarrhea since yesterday evening. My stomach hurt so much I couldn’t sleep. When I took my temperature this morning I had a slight fever at 37.5 degrees. Since this morning I’ve only been able to keep water down because I throw up whenever I eat.
昨日の夕方から、嘔吐と下痢が続いています。お腹が痛くて眠れませんでした。今朝、体温を測ると37.5度と微熱がありました。今朝から、食べるとすぐに吐いてしまうので、水しか口にできません。
下痢は、どのような便ですか。水のような便ですか。血は混じっていますか。
What is the diarrhea like? Is it watery? Is there blood in the stool?
It’s watery. I don’t think there was any blood.
水っぽいですね。血は出ていないと思います。
わかりました。では、胸の音を聞かせてください。上着を上げてもらえますか。
I see. Now, let’s have a listen to your lungs. Could you lift up your shirt, please?
大きく深呼吸をしてください。はい、楽にしてください。胸の音は問題ありません。
Take a deep breath in for me. OK, you can relax now. Your lungs sound clear.
ではお腹の音を聞かせてください。ベッドに寝てください。お腹を出して膝を立ててください。痛いところはありませんか。
Now, let’s have a listen to your stomach. Please lie down on the bed. Lift up your shirt and bend your knees. Are you feeling pain anywhere?
恐らくウイルス性胃腸炎だと思います。下痢と嘔吐がひどいので今日は念のため採血をした上で点滴をしましょう。水分をできるだけ摂るようにしてください。整腸剤*を3 日分出しますので、1 日 3 回飲んでください。食事が取れる場合は食後に飲んでください。吐き気が続き食事が取れない場合はお水などでお薬だけ飲んでもかまいません。 2、3 日して症状がよくならない場合、今より症状が悪くなった場合は、また受診してください。ではこの後、処置室で点滴をして帰ってください。
What you have is probably viral gastroenteritis. Since your diarrhea and vomiting are severe, we’ll take your blood samples today just to be sure, and then give you an IV. Be sure to drink lots of fluids. I will prescribe a 3-day course of probiotics. Take them 3 times a day. If you can eat, take it after each meal. If you continue to feel nauseous and are unable to eat, you can take the medicine alone with water. If your symptoms do not get better in a few days, or if they get worse, please come back and see us. Now, please go to the treatment room for an IV drip and then you can go home after that.

Can I get this medicine at any pharmacy? There’s a big drug store near my house. Can I buy it there?
この薬はどこの薬局でも手に入りますか？家の近くに大きなドラッグストアがあります。そこで買えますか？
処方せんを取り扱っている調剤薬局でしかお薬は購入できません。そのドラックストアが処方せんを取り扱っているなら購入できます。病院の正面玄関を出た通りの向かい側にも調剤薬局がありますのでお時間があればそこで購入することもできますが、どうしますか。
You can only purchase the medicine from a dispensing pharmacy that handles prescriptions. If the drug store handles prescriptions, you can buy your medicine there. There is also a dispensing pharmacy across the street from the main entrance of the hospital, so if you have time, you can purchase your medicine there. What would you like to do?
I see. I don’t remember if the drug store by my house has a pharmacy or not, so I think I’ll go to the one across the street. Thank you for your help.
なるほど。家のそばのドラッグストアに薬局があるかどうかは覚えていないので、そこの向かい側のドラッグストアに行ってみようと思います。ありがとうございます。

A medicine notebook is a pocketbook for recording the name, dosage, frequency, and instructions for taking the medicines prescribed to you. Would you like one?
I don’t think I need one because I can’t read Japanese.
日本語が読めないので、必要ないと思います。
お薬の処方の重複や、飲み合わせの悪いお薬の処方を避けることができるので、作られると良いと思いますが …
I think it is a good idea to make one because it helps us avoid duplication of medicines and bad drug combinations.
That’s right, even if I can’t read it the pharmacist can. Okay, I’ll take one, please.

はい。このお薬はジェネリック医薬品も用意できますが、希望しますか。 ジェネリック医薬品とは、今回処方されたお薬と有効成分が同じで、価格が安いものです。
Okay. Would you like to have a generic version of this medicine? Generic medicines are the ones that have the same active ingredients as the medicine you have been prescribed but at a lower price.

お薬の準備ができました。質問表をいただきます。今、治療中の病気や服薬中のお薬、過去にアレルギーが出たお薬などはありますか。

I’m not taking any medicine or getting any kind of treatment. I have food allergies, but I am not allergic to medicines.

わかりました。お薬が 1 種類、3 日分出ています。 これは胃腸の働きを整えるお薬です。1日３回食後に飲んでください。 これがお薬手帳です。今回処方されたお薬についてはここに記録しています。 今後、調剤薬局に行くときは、持っていってください。お薬代は 900 円です。

通訳のポイント
*「整腸剤」は、調べると probiotics や antiflatulent などがあります。特にantiflatulent は「腸のガスを減らす」という意味があります。単語が思いつかなかったら、a medicine for intestinal symptomsなどと言い換える（パラフレーズ）ことも一つの手です。

まとめ
今回は「内科」を受診する外国人患者さんとその患者さんに関わる医療従事者の会話スクリプトです。
総合案内、診察室、会計、薬局と４場面でそれぞれ使用する用語があります。総合案内では医療機関で一般的に使われる用語、診察室では医療用語、会計では薬局の仕組みや数字、薬局ではお薬手帳やジェネリックの説明などに関する言葉が出てきました。
医療通訳は、医療英語ばかりを覚えていればいいわけではなく、さまざまな表現や用語を知っておく必要があります。２次試験の練習は、幅広くいろいろなシチュエーションを想定して行っておくといいですね。
